GENERAL PRACTICE SERVICES
At Playford Medical Centre, we provide comprehensive, patient-centred general practice care for individuals and families at every stage of life. Our GPs offer a full range of services including men’s, women’s and children’s health, wound care, and blood tests and pathology collection. We also provide administrative and legal medical services such as driver’s licence medicals, employment medicals, medical certificates, care plans, and referrals.
Our team has a strong focus on chronic disease management, supporting patients living with conditions such as diabetes, asthma, hypertension, cardiovascular disease, chronic kidney disease, arthritis, and chronic pain. We provide GP Management Plans (GPMPs), coordinated care plans, medication reviews, heart disease monitoring, and referrals to allied health professionals including dietitians, psychologists, physiotherapists, and diabetes educators to ensure a multidisciplinary approach tailored to each patient’s needs.
Preventive health is a key priority at our practice. We offer immunisations, health checks, lifestyle risk assessments, and screening services to help detect and prevent illness early. We also provide prompt acute care for everyday health concerns such as colds and flu, sore throats, urinary tract infections, skin infections and rashes, minor wounds and sprains, headaches, and abdominal pain. Our medical centre is committed to providing culturally safe and comprehensive healthcare for Aboriginal and Torres Strait Islander peoples. We offer a range of services including general health checks, Aboriginal and Torres Strait Islander Health Assessments (MBS Item 715), chronic disease management, preventative screenings, immunisations, women’s and men’s health services, mental health support, care coordination, and referrals to specialist and allied health services. We work collaboratively with patients, families, and community to support holistic wellbeing — recognising the importance of physical, emotional, cultural, and community health — and strive to ensure care is respectful, inclusive, and accessible for all.
SPECIAL SERVICES
Our medical practice is proud to partner with our local Primary Health Network (PHN) to deliver targeted programs that support some of the most vulnerable members of our community. Through our involvement in PHN initiatives focused on refugees, family and domestic violence, sexual abuse and violence, the Flourish program, and aged care services, we are committed to providing trauma-informed, culturally sensitive, and patient-centred care across all stages of life. These programs strengthen our ability to support individuals and families experiencing complex health and social challenges, while also enhancing our chronic disease management and comprehensive aged care services through coordinated, multidisciplinary care. By participating in these projects, we aim to improve health outcomes, increase access to essential services, and foster long-term wellbeing for our patients and the wider community.
